Understanding Children’s Teeth

Milk Teeth, Permanent Teeth and Mixed Dentition

Children have two sets of teeth:

Primary (milk) teeth – usually 20 in total
Permanent teeth – usually 32, including wisdom teeth

In most children:

All 20 milk teeth erupt by around 3 years of age
Milk teeth start falling between 7 and 12 years, as permanent teeth replace them
The first permanent molars usually erupt around 6 years – they are very important teeth that do not replace any milk tooth
By about 14 years, most permanent teeth have erupted (wisdom teeth may come later, between 18 and 25 years)

Between about 6 and 12 years, children have both milk and permanent teeth in the mouth. This stage is called mixed dentition.

Importance of Milk Teeth for Future Alignment

Milk teeth are not “just temporary” – they play a crucial role in:

Chewing and nutrition
Speech development
Facial growth and appearance
Maintaining space for permanent teeth to erupt in the correct position

If milk teeth are lost too early due to decay or extraction, nearby teeth can drift and block the space for the permanent successor, leading to crowding, misalignment or teeth getting stuck in the jaw.

At Child Smile Care, we focus on preserving milk teeth until the right age and protecting the developing permanent teeth underneath.

Common Pediatric Dental Problems

Cavities and Tooth Decay

Tooth decay in children often develops faster than in adults because:

Milk teeth are smaller and thinner
Children tend to snack more frequently
Brushing is often less effective without supervision

A major risk factor is prolonged bottle feeding, especially when a child sleeps with a bottle of milk or sweetened liquids. This can lead to nursing bottle caries—extensive decay affecting multiple front and back teeth.

We help parents:

Identify early signs of decay (white/brown spots, food trapping, sensitivity, pain)
Understand the causes of decay
Adopt preventive measures at home

“Double Teeth” – When Permanent Teeth Erupt with Milk Teeth

Sometimes parents notice two rows of teeth—a permanent tooth erupting behind or beside a milk tooth that has not yet fallen.

This may happen when the permanent tooth is not positioned directly under the milk tooth and erupts without resorbing the milk tooth root.

In most cases:

The situation should not be allowed to continue for more than a couple of weeks
Timely removal of the milk tooth allows the permanent tooth to move into the correct position

We assess each case carefully and advise when removal and/or monitoring is needed.

Treatments We Offer

At Child Smile Care, we provide a full range of child-specific dental treatments with a focus on comfort, safety and long-term oral health.

Tooth-Coloured Fillings for Children

When cavities are detected early, fillings can restore the tooth and prevent further damage.

Our fillings:

Tooth-coloured and blend naturally
Child-safe materials
Restore function & appearance

During treatment, we use:

Tell-show-do explanations
Gentle techniques
Short, manageable appointments

Crowns for Baby Teeth

If a tooth is extensively decayed or has undergone root canal treatment, a crown may be the best way to protect it.

Crowns for baby teeth:

Cover and strengthen weakened teeth
Help the child chew properly
Maintain space and support speech and jaw development

We guide parents on:

When a crown is truly necessary
What type of crown is suitable
How to care for crowned teeth at home

Root Canal Treatment in Milk Teeth

When decay reaches the nerve (pulp) of a milk tooth, it can cause pain, infection and swelling.

In many cases, the tooth can still be saved with pulp therapy or root canal treatment for children, followed by a crown.

Benefits include:

Pain relief
Preservation of the tooth until its natural shedding time
Protection of the underlying developing permanent tooth

Extraction is considered only when absolutely necessary or when the tooth cannot be saved.

Space Maintainers

If a milk tooth must be removed much earlier than its normal shedding time, we often recommend a space maintainer.

When a Space Maintainer Is Needed

A milk tooth is lost early due to decay, trauma or infection
The permanent successor is not expected to erupt soon
There is a risk of space closure due to drifting of adjacent teeth

How a Space Maintainer Works

Holds the gap open
Prevents shifting of neighbouring teeth
Creates room for the permanent tooth to erupt normally

Using a space maintainer correctly now can significantly reduce the need for complex and lengthy orthodontic treatment later.

Early Orthodontic Screening

Ideal Age for Orthodontic Check-Up:

Although many orthodontic treatments start around 12-13 years, early screening is recommended earlier, usually by 7 years of age.

Why Early Screening Matters:

It helps detect problems early
It can guide jow growth in the right direction
It reduces or prevents the need for extractions
It can shorten or simpilly future orthodontic treatment
It boosts the confidence of the child and parents

Early evaluation today can lead to a healthier, straighter smile and a more confident child tomorrow.

Highly recommend Dr. Swati! We took our 3-year-old for a cavity treatment, and the experience was seamless. Dr. Swati prioritised our child's comfort from the start, performing the checkup right on her mother’s lap. The entire treatment took just 10 minutes—completely painless and without tears. My daughter didn't even realize the filling was being done. Dr. Swati combines genuine competence with an exceptional, patient-first temperament. A rare and fantastic doctor. Highly recommend!

Dr. Swati Karkare is truly one of the most genuine, caring, and cheerful doctors I have ever met.

My son is 9 years old, and before the treatment he was extremely scared. Like most children, he was worried that it would hurt and that he would have to take injections, so he was not ready for the procedure at all. However, from the moment we entered the clinic, Dr. Swati made him feel comfortable. She engaged him in friendly conversations, kept him relaxed, and gradually took away all his fear.

Dr Swati Madam did filling and preserved his teeth. Her mastery in handling children on that fearful dental chair is at par. She also guided us nicely about dental care in future for prevention from getting cavities. Thank you Dr Swati Karkare madam. You are the best Pediatric dentist in Nashik.

Overall it’s been a delightful experience.. Dr Swati’s exceptional way of handling took away the nervousness out of my kids mind. My kids are very comfortable and although we stay in Navi Mumbai, they prefer taking consultation from Dr Swati rather than the nearby dentist. Thank you so much😊😊
